- Print your CSSPS placement form You will need it throughout registration and on reporting day.
- Have your enrolment code ready It is printed on your placement form and is required to continue.
- No code, no admission Your admission stays incomplete until the enrolment code is entered.

- Placement form (1 original + 1 photocopy)
- Personal Record form
- Admission Letter (2 copies)
- Birth Certificate / Baptismal / Ghana Card
- 2 Passport pictures
